Referrals

Individuals who wish to use our service can do so via referral from their Care Manager or Social Worker. Alternatively you can contact us directly.

All new and potential Service Users will meet with the service provider Rebecca Kelly to discuss your care and support needs, your expectations and to determine if our service is suitable for you.

We will also discuss the terms and conditions of our contact and tell you how much our services cost. Rebecca will also discuss important matters like safeguarding with you, giving your consent, sharing information and our procedure for raising concerns, paying us a compliment or making a complaint.

Accessible Information Standards

All organisations that provide NHS or adult social care must follow the Accessible Information Standard by law. This means Epiphany in care must provide information to you in a way that is best for your communication needs. 

This could mean providing information to you in:

  • large print

  • Brail

  • Audio or lip reading

  • Easy read format

  • Or translating information into another language

Please contact us direct if you need information from us in an accessible format. 

There are five main points of the AIS standards that all social care agencies must follow. These are:

1. Ask people if they have any information or communication needs, and find out how to meet their needs. 

2. Record those needs clearly and in a set way. 

3. Highlight or flag the person’s file or notes so it is clear that they have information or communication needs and how to meet those needs. 

4. Share information about people’s information and communication needs with other providers of NHS and adult social care, when they have consent or permission to do so. 

5. Take steps to ensure that people receive information which they can access and understand, and receive communication support if they need it.
